Vision

P2link vision is to decrease chronic disease costs by reducing patient re-hospitalization. World population is aging, with rising numbers of medical conditions and diseases. Societies are facing with related impacts on public health services and privately owned medical organizations.

P2link envisages creating simple and human centric actions supported by state-of-the-art technology wherever possible, independently of any social, economic, geographical or cultural factors.

Approach

P2link approach focuses on chronic diseases rather than on acute illnesses. Chronic diseases impact the patient & make up 80+ percent of total healthcare expenses. To deliver this promise, P2link has developed a full set of very powerful tools based on a multi-local approach and follow P2link high-level socio-medical standards.

Innovative strategy

P2link develops tools focusing on secondary prevention, to reduce the duration and frequency of re-hospitalizations of patients suffering from CHF and COPD, with a significant impact on the total costs of these two pathologies. An obvious solution is to reduce costs is… not to have to go to hospital and to optimize the length of stay. P2link has aligned all its innovative strategy to keep patients suffering from chronic diseases at home as long as possible.

Chronic diseases

P2link’s founding premise is to increase the quality of life of patients with chronic diseases while effectively contributing to lowering costs. The world population is growing and aging. Therefore, the typology of diseases is changing. Chronic diseases are becoming more and more common. We are targeting two main chronic diseases, Chronic Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD), sixty percent of the cost of treating is hospitalization.
